Issuance of a Medical Certificate (Including a Referral)
In accordance with Article 17, Paragraph 1 of the Medical Service Act, a medical certificate (including a letter of referral) is issued only to the patient unless the patient dies or is unconscious (excluding the issuance of a copy of the medical certificate issued in the past).
Note
- The International Healthcare Center does not prepare medical certificates for patients. When you see a doctor, be sure to ask directly for a certificate .
- If you do not ask for a medical certificate at the time of consultation but after the consultation, you must see the doctor again, as the International Healthcare Center
  does not ask the doctor for a medical certificate. Please note that the fee will be charged separately.
- Basically medical certificate is issued in Korean. If you need English medical certificate, ask for in English.
Documents for Accessing Medical Records and Issuing Copies
- You will need your ID (Alien Registration Card (ARC) and passport) to view medical records and issue copies.
- If a representative, other than yourself, requests for access and issuance, he/she must bring the following documents.

Required Documents (if the patient’s consent is not obtained)
- If the patient does not have any family members as prescribed by the Medical Service Act, the patient’s sibling can apply for the access of medical records and copy
  issuance upon submission of the required documents, and a document supporting the lack of family member for each situation.
- If a patient’s consent is not available, a legal representative or arbitrary representative of a family member and a family member, who has the right to issue copies, can
 also apply (e.g., documents required when applying for issuance of a copy of the deceased patient to a third party such as Alien Registration Card (ARC), identification card of
 family member, power of attorney between family member and representative, applicant [representative]’s identification card).
The family member of the patient (spouse, lineal descendant, and
direct descendant or the spouse’s lineal descendant) can apply.
If the patient is deceased
- ID card or copy of the applicant (family member)
- A certificate of kinship with the patient (Alien Registration Certificate / 외국인 등록 증명서)
* The Alien Registration Certificate must show the relationship with the patient.
- Confirmation of death
 (e.g., Death Certificate, Family Census Register [Including Cancelled or Removed Person], etc.)
If the patient is conscious or unconscious,
but unable to sign his/her signature due to
a serious illness or injury
- ID card or copy of the applicant (family member)
- A certificate of kinship with the patient (Alien Registration Certificate / 외국인 등록 증명서)
* The Alien Registration Certificate must show the relationship with the patient.
- A medical certificate confirming that the patient cannot sign his/her signature due to unconsciousness,
 or serious illness or injury.
If the patient is missing
- ID card or copy of the applicant (family member)
- A certificate of kinship with the patient (Alien Registration Certificate / 외국인 등록 증명서)
* The Alien Registration Certificate must show the relationship with the patient.
- A document confirming the patient’s missing status (a copy of the court’s missing notification decision, etc.)
If the patient is mentally incapacitated
- ID card or copy of the applicant (family member)
- A certificate of kinship with the patient (Alien Registration Certificate / 외국인 등록 증명서)
* The Alien Registration Certificate must show the relationship with the patient.
- A document confirming the patient’s mentally incapacitated status (a copy of the court’s decision
 that the person is incompetent, an injunction order of the court, a medical certificate
 by the department of psychiatry, etc.)

About the Issuance of the Copy of Medical Record and Image
- In accordance with Article 21 of the Medical Service Act, the patient must come to the hospital (or with the necessary documents) to request
  a copy of the medical record. Please visit the International Healthcare Center with your ID and fill out a Medical Record Request Form.
- If a representative other than yourself, applies for the copy, he/she must bring the following documents. * Please see the table below.
- The consent form and power of attorney must be original and must be signed by the patient.
- Seal and thumbprint will not be accepted.

- Your signature must be in print, so that your name can be easily recognized by third parties.
- Only the original required documents (Alien Registration Card (ARC), etc.) within 3 months of the date of issuance are recognized.
Required Documents for the Issuance of a Copy of the Medical Record and Image
- To obtain a copy of the medical records and images, the patient’s ID (Alien Registration Card (ARC) and passport) is required.

A copy of the medical record includes the following
1. Doctor’s instruction sheet: Initial record, progress record, admission and discharge summary, surgical record, emergency record, etc.
2. Result: Test results (blood, urine, etc.), radiological results (CT, X-ray, MRI, etc.)

Process of the Issuance of the Image Data (CD) Copy

Note on the Preparation of Required Documents
- The issuance process of the relevant required documents is the same as that of a copy of the medical records.
- If you need a copy of the medical records and image data, please apply according to the issuance process of the medical record copy.
  The image data at the time of the issuance of the medical record copy will be issued if deemed necessary.
- Image data requested by the medical department: Echocardiography, cardiovascular angiography, ophthalmologic examination, and electroencephalography.
- Image data requested by the radiology department: CT, MRI, X-Ray, PET-CT, ultrasound, special examination, nuclear medicine examination,
  endoscope, retinography, etc.

- Closed on holidays
- It may take a long time due to multiple copies.
