In accordance with Article 17, Paragraph 1 of the Medical Service Act, a medical certificate (including a letter of referral) is issued only to the patient unless the patient dies or is unconscious (excluding the issuance of a copy of the medical certificate issued in the past).

- This refers to a document that proves the patient’s admission to or discharge from the hospital.
- This refers to a document that records the findings after the doctor examines the patient, including the patient’s personal information, the date of onset, the name of the disease, the date of diagnosis, and the opinion of the doctor.
- This refers to a document confirming that the outpatient medical treatment was performed due to illness or injury.
- This refers to a document that proves the fact of birth, which includes the birth name, the date and place of birth, and the parents’ personal information.
- Your signature must be in print, so that your name can be easily recognized by third parties.
- Only the original required documents (Alien Registration Card (ARC), etc.) within 3 months of the date of issuance are recognized.
