Konkuk University Medical Center is a major cardiac and aortic referral center where more than 80 major cardiac and aortic surgical procedures are performed each month. The formal teaching program of KUH-Cardiac Anesthesia Service includes participating in many of case-sensitive patient care, accumulating experience with 2-D and live 3-D transesophageal echocardiography (TEE), and learning from directed reading and topical reviews, formal case discussions, staff meetings, case presentations, lectures by staff and visiting professors.
Clinical Research is an active and extensive part of the division's efforts and is with special emphasis on anti-inflammatory strategy, fast-tracking anesthesia and various intraoperative hemodynamic monitoring.
The staff is comprised of 3 full time board-certified anesthesiologists. The staff is internationally known in their efforts to use of intraoperative TEE, continuous cardiac output monitor, cerebral oximetry and arterial-pressure derive cardiac output monitor, for the assessment of risks, guidance of therapy, and education of other cardiovascular anesthesiologist throughout the world.

Clinical Case Mix in KUMC-Cardiac Anesthesia Service

KUMC serves as a tertiary care center for performing cardiac and aortic arch surgery, where the most critically ill patients are referred often after being rejected by other hospitals. Caseload at KUMC is varied, and on the cutting edge of technological development in the field of cardiac surgery. All types of procedures are performed here including cardiac transplantation in two operation rooms equipped with globally standardized facilities. ?While intraoperative TEE is performed on every cardiac case, we employed intraoperative live 3D TEE for cardiac cases for the first time in Korea. KUMC performs a large number of mitral and aortic valve repair surgery requiring intraoperative TEE determination of pre- and post-repair valve function. In addition, most cases of aortic arch surgery in Korea are performed in KUMC on a 24 hour base. KUMC-Cardiac anesthesia service also provides anesthesia for the cardiac transplantation surgeries. Aortic arch surgery is an integral part of our practice. We utilize optimal techniques of anesthesia with all type of patient monitor and equipment for aortic arch surgery, including intraoperative blood salvage, lung isolation techniques using Univent tube and fiberoptic bronchoscope, application of thromboelastography for coagulation profile, optimization of intravascular blood volume using rapid infusion system. The anesthesiologist's role in these cases is extensive, and we have been actively involved in educating anesthesiologists at other hospitals in the management of these cases. |

Cardiac Anesthesia Fellowship Program with Perioperative Echocardiography
Fellowship positions are offered for one to two years Fellowship programs in cardiac anesthesia, cardiac postoperative care, and perioperative echocardiography at the KUMC-Cardiac Anesthesia Service where about 1000 open heart procedures are performed anually. Case mix includes major valvular repairs, adult complex congenital surgery, multi-organ transplantation, aortic arch surgery, and aortocoronary bypass procedures. Perioperative echocardiography training will include clinical and research experience with both transesophageal and epiaortic echocardiography. Intraoperative echocardiography service has been performed in all cardiac surgery case in KUMC.

Staff level preoperative assessment of the patient with cardiopulmonary disease
Evaluation of anesthetic risks and necessary interventions to minimize risk
Determination of cardiovascular and pulmonary monitoring requirements and acquire expertise in invasive monitoring techniques including TEE
Design and administration of anesthetic medications and/or regional anesthetics for optimal surgical conditions, with consideration of patient disease processes
Effective perioperative management of the cardiac surgical patient with complex cardiovascular pathophysiology based upon knowledge and consideration of cardiac, respiratory, neurologic, renal, hepatic, neuromuscular, hematologic and coagulation systems
Preoperative, intraoperative, and postoperative management of patients with coronary artery disease, valvular disease, congenital heart disease, cardiomyopathies, transplants, dysrhythmias, thoracic and aortic disease
Management of the patient that is difficult to wean from cardiopulmonary bypass
Coagulation management during and after cardiopulmonary bypass
Understanding the principles of extracorporeal circulation, cardiopulmonary bypass and circulatory assist devices
Understanding the principles of myocardial preservation
Effective management of routine and ill patients in the postoperative cardiac intensive care setting
Critical review of the cardiovascular literature
Participation in didactic sessions (e.g., Anesthesiology Grand Rounds), clinical teaching, and presentation at cardiovascular journal club
Ability to formulate clinically relevant hypotheses in cardiac and thoracic anesthesiology and design studies to effectively test such hypotheses according to the scientific method
Execution of research projects, interpretation of results, and preparation of abstracts and manuscripts

Simulation training for anesthesia residents in KUMS
|
|
In Konkuk University Medical Simulation Center(KUMS), the state-of-the-art equipments and interactive mannequins including Laedal Simman¢â will provide new training opportunities with remarkably lifelike simulators for medical students, nurses, medical rescuers and doctors. For example, they can re-enact clinical problems such as having an advanced cardiac life support, difficult airway management, difficult airway management, etc. The simulators provide the maximum education out of scarce opportunities, especially in high risk and low volume areas of critical care, anesthesia and emergency medicine.
With the new simulators, KUH Anesthesia department aims to enhance medical expertise, patient safety and positive outcomes in the long-run.
All anesthesia residents are taking simulation training before starting their curriculum and continue training using various simulation senarios.
